Could the Mediterranean Diet Improve Fertility?
For decades, fertility has been viewed through the lens of hormones, age and genetics. But what if one of the most influential factors was sitting on your dinner plate?
Growing research suggests that Mediterranean diet may create a healthier environment for conception, particularly for people trying to conceive naturally or through assisted reproductive technologies such as IVF.
Your diet cannot guarantee pregnancy, but it can influence the environment in which eggs, sperm and embryos develop.
What is the Mediterranean Diet?
The Mediterranean diet is a traditional dietary pattern based on the eating habits of countries bordering the Mediterranean Sea. Rather than focusing on restriction, it emphasises whole, minimally processed foods that nourish the body over the long term.
Key features:
- Plenty of fruits and vegetables
- Whole grains
- Legumes
- Nuts and Seeds
- Extra virgin olive oil
- Moderate amount of seafood including fish
- Moderate dairy intake
- Limited red and processed meats
- Minimal ultra-processed foods and refined sugars
What does the research show?
For Females
Several studies found that women with greater adherence to the Mediterranean diet experienced:
- Higher clinical pregnancy rates
- Higher live birth rates
- Improved ovarian response during IVF
- Greater embryo yield
In one study it was found that among non-obese women undergoing their first IVF cycle, those who followed a Mediterranean diet more closely during the previous 6 months were more likely to achieve a clinical pregnancy and live birth. Every 5 point increase in the Mediterranean diet score was associated with 2.7 fold higher chance of pregnancy and live birth.
Although many studies have been conducted in people undergoing IVF, these findings provide valuable insight because IVF allows researchers to directly measure egg quality, embryo development and implantation, processes that are music harder to study in natural conception.
For Males
Among men, higher Mediterranean diet adherence has consistently been associated with healthier semen quality.
A study found that men with the lowest Mediterranean diet score were approximately 2.6 times more likely to:
- Have abnormal sperm concentration
- Have an abnormal total sperm count
- Have poor sperm mobility
How might the Mediterranean diet support fertility?
1. Reduced Inflammation
- Inflammation can impair sperm production and function, reducing inflammation may improve semen quality.
- Low grade chronic inflammation can impair ovulation, implantation, placental development and sperm function.
- The Mediterranean diet is rich in anti-inflammatory foods such as extra virgin olive oil, fatty fish, nuts, seeds, fruits, legumes and vegetables. These foods help lower inflammatory markers, creating a uterine environment that is more favourable for embryo implantation and early pregnancy.
2. More Antioxidants = Reduced Oxidative Stress
- Oxidative stress occurs when harmful free radicals overwhelm the body’s antioxidant defences.
- Antioxidants help protect sperm and eggs from oxidative stress, which can damage sperm DNA and reduce sperm motility
- The Mediterranean diet provides antioxidants such as polyphenols, vitamin C, vitamin E and carotenoids
3. Improves Metabolic health
- In a woman, insulin resistance can disrupt ovulation and contribute to conditions such as polycystic ovary syndrome (PCOS).
- In men, obesity and metabolic dysfunction have been associated with lower testosterone levels, impaired sperm production, and poorer semen quality.
- The Mediterranean diet emphasises whole grains, beans, Lentils, vegetables and healthy fats which help stabilise blood sugar and insulin levels.
4. Supports healthy hormone production
- Healthy fats are key for the production of steroid hormones including testosterone, oestrogen and progesterone.
- The Mediterranean diet supports the production of reproductive hormones due to the emphasis of health fats such as monounsaturated fats like olive oil and omega-3 fatty acids from fish, nuts and seeds.
5. Supports sperm structure and function
- Fish and seafood provide omega-3 fatty acids, particularly DHA
- These fats are important because sperm cell membranes naturally contain high concentrations of long chain polyunsaturated fatty acids, which help maintain sperm structure and function.
6. Support health methylation
- Methylation is an essential process that regulates gene expression during egg maturation, sperm development and early embryo growth
- Proper methylation is particularly important during the first few days after fertilisation when the embryo undergoes extensive epigenetic programming
- Nutrients abundant in the Mediterranean diet including folate, vitamin B12, vitamin B6 and choline provide building blocks needed for methylation
What does this mean in practice?
You don’t need to eat a perfect Mediterranean diet overnight. Small changes can make a meaningful difference over time, such as:
· Choosing extra virgin olive oil instead of butter
· Eating oily fish twice per week
· Including legumes several time per week
· Eating a wide variety of colourful vegetables n
· Choosing whole grains over refined carbohydrates
· Replacing processed snacks with nuts and fruit
While no diet can guarantee pregnancy, growing evidence suggests that the Mediterranean diet creates a healthier biological environment for conception by reducing inflammation, protecting against oxidative stress, supporting hormone production and improving metabolic health.
